- Vaccinate Now
- Discipline Policy
- Suspension Form
- Attendance Regulations

Proof of age (Original Birth Certificate)
Proof of required immunizations
School transcript, report card
Special Education IEP (if applicable)
Custody papers (if applicable)
Proof of Residency (Current electric bill if you own your home; Copy of lease if you rent)

Forms to be completed for specific grade levels:
- Physical Examination Form - required for each student upon entry into school (Kindergarten or First Grade) and in Grades 6 and 11
- Dental History - required for each student upon entry into school (Kindergarten or First Grade) and in Grades 3 and 7
