Registration > Registration for Children Entering Kindergarten in September 2008 to be held on February 6th, 2008

Souderton Area School District kindergarten registration will be held on Wednesday, February 6, 2008 at the seven elementary schools within the district as follows:

• E.M. Crouthamel Elementary School 9:00-11:00a.m.
• Franconia Elementary School 9:30a.m. & 1:30p.m.
• Lower Salford Elementary School 9:00-11:00a.m.
• Oak Ridge Elementary School 9:00a.m.-12:00p.m.
• Salford Hills Elementary School 9:30-11:00a.m.
• Vernfield Elementary School 9:30-11:00a.m.
• West Broad Elementary School 9:30-11:00a.m.

In early January, parents in Souderton Area School District with children eligible for kindergarten in September 2008 (as identified by the census records) will be receiving a letter from the principal of the elementary school where their child will attend. Parents will receive a number upon arrival to the school indicating the order of registration. It is not necessary for them to bring their child(ren) to registration.

Enclosed with the letter, parents will receive a number of forms, some of which need to be completed before the day of registration. The forms are as follows:

• Registration Form
• Developmental History Form
• Home Language Survey Form
• Parental Consent for Use of Image(s)
• Health History Sheet
• School Health Services Permission Form
• Parent Questionnaire
• Montgomery County Health Department Immunization
• Attendance Regulations
• Emergency Information Card

Parents are requested to bring a copy of their child's immunizations to registration. The following are required for entrance into school:

• Diphtheria and Tetanus - 4 doses with 1 dose administered on or after the fourth birthday
• Polio - 3 or more properly spaced doses
• Hepatitis B - 3 properly spaced doses
• Measles, Mumps, Rubella - 2 properly spaced doses, with first dose given at 12 months of age or older, followed by recommended MMR2
• Varicella (chicken pox) immunity

Children who will be five years of age before September 1, 2008 will be eligible for kindergarten. Parents should bring a copy of their child's birth certificate to the registration.

Parents will help expedite the process by completing all forms before arriving at registration.
